Objective:  To monitor and care for patients with chronic conditions and strive to improve their clinical outcomes.

Essential Functions:

  • Provides care management for patients with chronic conditions.
  • Provides systematic assessment of the patient's medical, functional, and psychosocial needs.
  • Ensures timely receipt of all recommended preventative care services.
  • Performs medication reconciliation with review of adherence and potential interaction.
  • Creates a patient-centered care plan based on a physical, mental, cognitive, psychosocial, functional, and environmental (re)assessment and an inventory of resources and supports.
  • Communicates the comprehensive care plan with other clinicians and providers, as needed.
  • Provides patient with the written and/or electronic copy of his/her comprehensive care plan.
  • Utilizes the patient portal to deliver the care plan and encourages all patients (or their designated caregiver) to join and become familiar with use of the portal.
  • Document the time spent on patient services.


Preferred Skills:

  • Basic computer skills in Microsoft Word/Excel.
  • Must be able to type at least 40 wpm.
  • Strong verbal and written communication skills.
  • Ability to work with minimal supervision.
  • Familiarity with electronic health records required.


  • Registered Nurse (RN), Licensed Practitioner Nurse (LPN), or Certified Medical Assistant (CMA) preferred.
  • Minimum of 1 year of clinical experience preferred.
